  Typically the ground wire would be connected to the frame of the trailer using ground screws like # FA1131822 and a ring terminal like part # DW05740-1. Using heat shrink is always recommended to help prevent corrosion. Connecting the ground to the axle technically would work, but the frame will be a much more secure location.   It is odd that the Malone instructions for the trailer # MPG464-LB do not mention connecting the ground anywhere. On a trailer like this, the white wire can be grounded on the steel frame (usually near the tongue) using a self tapping screw # FA1131822 and a ring terminal # DW05737-1. Preferably an out of the way location where it is out of the elements. Make sure it is grounded to good clean metal, free of any corrosion. A bad ground would certainly explain the issue with your lights.
